Crescent Medical Center LLC is a licensed primary care practice in the State of Rhode Island. Our main clinic is located in Johnston, RI, with an additional location in North Smithfield:

3. SMS / Text Messaging Terms

Crescent Medical Center uses text messaging to communicate with patients regarding appointments and administrative matters. By providing your mobile phone number to our office, you agree to the following:

• Consent: You expressly consent to receive SMS text messages from Crescent Medical Center at the mobile number you provide. Consent is not a condition of receiving care.
• Message types: Text messages from our office may include appointment confirmations, appointment reminders, scheduling changes, and general administrative notices. We do not send promotional or marketing texts.
• Frequency: Message frequency varies and is based on your scheduled appointments and communications with our office. You will not receive unsolicited or excessive messages.
• Opt-out: You may opt out at any time by replying STOP to any message from our office. You will receive a single confirmation and no further messages will be sent. To re-enroll, contact our office directly.
• Message and data rates: Standard message and data rates may apply based on your mobile carrier plan. Crescent Medical Center does not charge for text messages.
• No protected health information via SMS: Text messaging is not a HIPAA-secure communication channel. Do not send medical information, symptoms, test results, or other protected health information (PHI) via text message. For all clinical matters, please call our office directly at (401) 227-9036 or use our secure patient portal.
• Carrier disclaimer: Mobile carriers are not liable for delayed or undelivered messages. If you do not receive an expected message, please contact our office by phone.

4. No Doctor-Patient Relationship

Using this website, submitting a contact form, or exchanging text messages with our office does not establish a doctor-patient relationship. A formal care relationship is only established after an in-person evaluation at one of our clinic locations and completion of our intake process.

Nothing on this website constitutes medical advice, diagnosis, or treatment. Always consult a qualified physician for medical concerns. If you are experiencing a medical emergency, call 911 immediately.

5. Appointment Requests & Cancellation Policy

Submitting an appointment request through our website does not guarantee an appointment. All requests are reviewed by our staff and confirmed within two business days. Appointment availability is subject to provider schedules and clinic capacity.

No-show / late cancellation: Appointments cancelled with less than 24 hours' notice, or missed entirely, may be subject to a $25 no-show fee.

New patient intake forms: Our intake packet should be completed and either brought to your appointment in person or faxed to (401) 226-0854.

6. Patient Information & Privacy

Any personal information submitted through this website is handled in accordance with our Privacy Policy. Protected health information shared during your clinical care is governed by our HIPAA Notice of Privacy Practices, available at our front desk.

7. Intellectual Property

All content on this website — including text, logos, images, and design — is the property of Crescent Medical Center LLC and may not be copied, reproduced, or distributed without written permission.

8. Third-Party Links
